Academic Scholarship Requirements




2008 - 2009 Scholastic Year


Deadline for Completed Application: April 15, 2008

Please read application carefully. Answer all questions truthfully and accurately. Provide all requested information to the best of your knowledge. Incomplete applications may be rejected. Please print or type all answers.

The following requirements must be met by all applicants:

  1. Permanent residency status and California residency of at least one year.
  2. Completion of a research paper on any aspect of Polish history or culture. The paper is not required submittal with the scholarship application. The PUC Scholarship Committee will contact the candidates and inform them that their paper is to be completed by a certain date.
  3. College sophomore or better status as of September 1, 2007.
  4. Attendance at two Club fundraisers for check presentation (usually held in mid-summer and mid-winter). Arrangements may be made for students attending schools outside the Southern California area.
  5. Attendance at a personal interview. Under special circumstances, the Scholarship Committee may allow an applicant the option of preparing a video according to a set of guidelines that will be provided to him/her. Interview (or video) is mandatory and will be granted only to those students satisfactorily meeting scholarship requirements. Interviews are held once a year only.
  6. Attendance at an accredited 4-year college or university issuing a bachelor's or higher degree.
  7. Applicants must become student members and pay the $10 annual student membership.
  8. STUDENTS MUST HAVE AT LEAST A 3.0 GPA FOR CONSIDERATION.

You are required to submit the following documents:

  1. Your most current transcript (post high school).
  2. Two letters of recommendation from your college professors, on college letterhead, signed and dated, mailed directly to the Chairman of the Scholarship Committee (address below).
  3. One letter of character reference from a responsible person who is a respected and established member of the community. You may not use members of your immediate family, other applicants or children.
  4. A recent identifiable, passport-size photograph.
  5. A copy of your most recent tuition payment (invoice only) for the quarter or semester. This will be held in strict confidence.
  6. A concise, informative and factual autobiography. The autobiography should be no longer than 500 words and should not exceed 1 page single-spaced.

Eligibility for scholarships is based upon character, scholastic standing and financial need. College tuition, fees and book costs may be paid with the grant. Half the grant will be given for the Fall 2007 semester and half for the Spring 2008 semester, provided all obligations are met. The amount of the scholarship is contingent upon the financial endowment received by the Dr. Thomas M. Potasz Memorial Trust Fund and Club fundraising activities.

Students may apply for the leadership and the academic scholarships concurrently. In such a case, the student needs to complete the application only once.
